We are providing data to NHS Digital as part of the General Practice Data for Planning and Research data collection, so that patient data can be used to help improve everyone's health and care. If you would like to opt out please download and complete a Register your Type 1 opt out preference form and return it to your surgery.
Confidentiality
The Law strictly controls the management of all personal information and all patient information is kept strictly confidential. Therefore your signed consent is always required before any information can be released to any third party who is not a health professional.
If you are concerned when attending the surgery that confidential information may be overheard and need to speak in private, please inform our receptionist.
