How to Register as a Patient

Register your New Baby

Please register your new baby at the practice as soon as possible. Download and fill out the Register with a GP Surgery (PRF1) form, or pick up a copy from reception. Return the completed form to reception and include your baby’s NHS Number, which can be found in their Red Book.

Once registered, we’ll send you an appointment for a mother and baby post natal check. This will take 50 minutes; 30 minutes for your baby’s check and 20 minutes for yours. Your baby’s immunisations will start at 8 weeks and will be done at this appointment.

Don’t forget to bring the Red Book. If you can’t make the appointment, let reception know as soon as possible. Please note, rescheduling may mean waiting up to 4 weeks for the next available slot.

Temporary Residents

The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to bring your passport and a completed Temporary Resident Registration (GMS3) form to the surgery. The form is also available at reception.

An NHS number is a 10 digit number, like 485 777 3456.

Your NHS number is unique to you. It helps healthcare staff and service providers identify you correctly and match your details to your health records.

Accountable Named GP for All Patients

All patients registered at Millbank Surgery have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the provision of your healthcare. This does not mean you can only see this GP. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

If you are unsure of who is your named Doctor please ask at reception.

New patients are allocated a Doctor on Registration.
