The practice catchment area is indicated on the sketch diagram below and is restricted to Central Stafford areas only. For further details regarding the areas covered by our practice, please ask at reception.

New Patients

Applicants to the practice list will be asked to complete an application form which will be considered by the partners prior to acceptance to the list. The form can be obtained from reception, or a copy may be or you can download a copy of the 'Registration Form' here.

If your application is refused you will be notified in writing with the reason for this, along with a contact telephone number to obtain details of primary care medical services in the area.

Upon being accepted to the practice list, you will receive a letter asking you to book a 'new patient check' appointment with the Health Care Assistant. This will include all family members aged 15 years and over who are able to attend the surgery.

Please be aware that acceptance onto our list is conditional upon all applicants attending this initial Registration Appointment.

Temporary Residents

The Doctors are happy to see patients who are temporarily resident in the Practice area should medical advice be required. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to bring your passport and complete our Temporary Resident Registration form which is available from reception, or can be downloaded from the 'Registration Forms' section on this page.
